Theres already a specific bloodbourne trainer out that you can mod with. Ive seen it over at Psxhax

Im pretty sure the offsets are still the same so if you look into it a little and change the api and jkpatch in the trainers files it should be good.

You can do one thing.. on your ps4 backup save data from the game you dont know CUSAxxxxx to exFAT usb/HDD.. then you can see exact name on your PC
